The Gynecology Medical Home RN serves as the key individual in gynecology (GYN) specialty clinic assisting the Gynecologist in creating a comprehensive Gynecological Medical Home (PACT principles in GYN care similar to a Perioperative Surgical Home). The RN works to support the Gynecology Clinic as part of the Women's Health Program in collaboration with other departments. Some travel between the East Orange and Lyons campus is required. The RN actively participates in initiatives such as Patient Centered Care, I CARE and High Reliability Organization (HRO). The RN is responsible for direct and indirect patient-centered care across the continuum with an emphasis on gynecological services. The RN will coordinate patient care in the outpatient clinic collaborating with multiple VA departments and community providers. The RN will work independently as a liaison and advisor to clinical and administrative staff to assure treatment and services are clearly documented according to VA Policy. The functions of the RN include but are not limited to: Working dynamically and cohesively with Gynecology providers and clinic staff, Women Veterans Program Manager, Mammography Coordinator, Women's Health Care Coordinator and Office of Community Care for implementation of programs to support Women's Health to ensure compliance of VHA Services for Women Veterans as outlined in VHA Handbook 1330.01. Ensures that all women Veterans undergoing gynecologic procedures or complex gynecologic work-ups have comprehensive pre and post procedure care and assists with communication and hand-offs back to the Women's Health-PCP and other members of the care team. Monitors clinic utilization and daily patient flow. The RN reviews internal and Community Care consults and facilitates scheduling in a timely manner. The RN triages telephone calls, and secure messaging and informs patients of lab and imaging results. The RN provides care coordination for gynecological consultations, pre-operative and post-operative teaching, and performs clinical assessments and interventions for women seen in the Gynecology Clinic. Functions as a navigator/case manager and liaison for planning, directing, and coordinating care for women Veterans receiving gynecologic care via Community Care and within the Medical Center. The RN will assist Veterans with consult coordination to other VA facilities. The RN monitors and ensures patient satisfaction among women Veterans, developing effective mechanisms and provides feedback to improvement of services. The RN assists with ensuring highest quality gynecological care for women Veterans including population health and preventive care and supporting Veteran self-care. Assists in the monitoring of delivery and coordination of care, and outcomes of services delivered and participates in committees for the purposes of strategic planning and monitoring the quality of care provided to women. The RN is an ad hoc member the Women Veterans Health Committee and provides updates and reports as requested. The RN participates in quality improvement programs and initiatives including system redesign, to improve access, quality, efficiency, and patient satisfaction. The RN actively looks for opportunities to improve service delivery and will provide feedback to the WVPM and Women Veterans Health Committee. Remains current with new developments in women's health and maintains professional competency through reading professional journals, engaging in continuing education activities, and professional learning experiences. Collaborates with the ACOS of Ambulatory Care and Chief of Surgery to provide excellent quality of care to women Veterans in a seamless environment that is gender specific and respectful of the needs of women Veterans and performs other duties as assigned. The incumbent is directly accountable to the Women Veteran Program Manager through the Women Veteran Medical Director. The Women Veteran Program operates from the Director's Office through the Chief of Staff. Work Schedule: Monday - Friday 7:30 AM to 4:00 PM Financial Disclosure Report: Not required
