The overall goal of the Women’s Health Navigator is to support our community in navigating AdventHealth Zephyrhills-Dade City services. The Women’s Health Navigator serves as a community resource for women’s health; assuring patients have appropriate and timely medical care and supportive services on an ongoing basis. The Women’s Health Navigator ensures patients are effectively and efficiently navigated through the health care system receiving appropriate referrals, support and educational services through their journey. The focus is on patient education ensuring obstetrics, gynecology, breast care, specialty therapies and Women’s Health standards are implemented and result in excellent patient outcomes. In addition, this position supports the development of congregational health, community outreach and marketing programs for the hospital. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
Main contact for providers, patients, and consumers.
Responsible to connect the consumer with a hospital service through a provider or self-referral.
Able to show and report navigation conversion activity to business development team.
Able to demonstrate creativity in opportunities to connect the consumer with a Women’s Health service.
Acts as an owner of the Women’s Health PRC platform. Maintains content and outcome understanding.
Assists Baby Place with tours patients, families, and/or significant others whether pre-delivery, GYN, and/or breast health needs as needed.
Connects patients and patient candidates to appropriate OB, GYN, breast health and specialty therapy care providers.
Works closely with Women’s Health Medical Directors for both campuses. Striving to achieve or maintain accreditation and quality initiatives for better outcomes.

Acts as a liaison between the hospital and patients, physician office staff, patient’s families and/or significant others through routine communication to determine the customer’s need and satisfaction with services provided. Reports patient, family, and/or significant other concerns to Department Leadership as indicated for resolution.
Follows up with patients via transition to home to implement no-discharge activities and enhance patient experience.
Communicates with courtesy and respect when interacting with internal or external customers.

Assists with facilitating an evidence-based, multidisciplinary, multispecialty approach to care throughout the care continuum.
Collaborates with regional AHs to support appropriate navigation of patients (needs ranging from minimal to complex) from the diagnostic phase through treatment planning, medical management and surveillance assuring patient have appropriate, timely, medical and supportive services.
Serves as invited as the expert to other regional AHs with current evidence-based practice and new emerging trends in Women’s Health.
Develops appropriate relationships to maximize patient experience across regional hospital system, campus and physician/ provider lines.
Demonstrates collaboration and teamwork with leaders in managing patient flow.
Works with leaders/educators across system to ensure excellence in staff and patient education.
Adheres to organizational standards for dress and professional appearance.
Considers factors related to safety, effectiveness, cost and impact on practice in the delivery of women’s health services.

Develops partnership with Business Development team to support growth of Women’s Health
KNOWLEDGE AND SKILLS REQUIRED:
Demonstrates ability to communicate professionally in the English language: verbally with a pleasant speaking voice and in the written language
Demonstrates strong organizational skills
Skilled in providing appropriate Breast health, GYN and OB education and program development.
Skilled in communications with multidisciplinary teams, patients, families and surgical teams.
Skilled in program development.
Experienced in community education & public speaking.
EDUCATION AND EXPERIENCE REQUIRED:
Bachelor in Healthcare or Business
5 or more years of experience in Program Development
