The Women's Health Community Care Clinical Coordinator (WHCC) Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. In addition, assesses, plans, implements and evaluates care based on age and gender specific components as well as assuming responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Duties include: Reviews processes and procedures by establishing policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in program level quality improvement processes and initiatives as well as customer service programs. Supports and actively participates in initiatives such as Patient Centered Care and I CARE. Supports the network and VHA mission under the guidance of the Women's Health Program with ongoing collaboration with Women's Health and Community Care. Serves as a coordinator in a multi-disciplinary environment interacting directly with outpatient and inpatient departments, Business Office and contracted or affiliated hospitals which provide care for VA patients. Assesses and monitors routine Women's Health services including but not limited to breast, maternity, gynecology and infertility services. Responds to breast cancer screening and other breast imaging reports, including the Breast Imaging Reporting and Data System (BIRADS) and community care processes/consult management categories. Ensures community provider(s) plans are communicated back to the VA health care teams along with obtaining medicalrecords in efforts to promote a smooth transition for the Veteran between services.
